*Requirements for new accounts: We will need the Membership card filled out
completely, the Patriot Act form signed, and three forms of Identification.
If you are mailing in your application please have the application notarized and
include photo copies of your identification**. We also will need to know how you
are eligible for membership. This would include a copy of a paycheck if
you work for the school system or a copy of your child's report card, if your
child attends VBCPS.

If you are joining because a family member referred you, please list that
persons' name on your application.
**Valid
Identification include a state issues driver's license or Identification Card,
military I.D., Social Security Card, or Paycheck. If you are not sure, call. We
do need Identification with your social security number pre printed on it.
